This AGREEMENT (“Agreement”) is entered into as of the day of , 2016 (the “Effective Date”) by and between Trinity Integrated Care, LLC, a Delaware limited liability company (“Company”), and the entity or individual identified on the signature page hereof (“ACO Participant”), on behalf of itself and, if applicable, those ACO Providers/Suppliers (as hereafter defined) that bill pursuant to a taxpayer identification number assigned to ACO Participant. The only parties to this Agreement are the ACO Participant and the Company.
