THIS MSO DEALER AGREEMENT (this “Agreement”) shall be deemed to be effective as of THE LATEST DATE SET FORTH ON THE SIGNATURE PAGE HERETO (the “Effective Date”), between DSI SYSTEMS INC, an Indiana company (“DSI”), and THE MSO DEALER LISTED ON THE SIGNATURE
