This Data Use Agreement (the “DUA”), dated as of the Effective Date, is entered into between the Association of Public Health Laboratories, Inc. (“APHL”), a nonprofit corporation organized under the laws of the District of Columbia, and [****Insert Name of Participating Jurisdiction****] (the “Participating IIS Jurisdiction”), an executive agency of the [****Insert City, County, State, Tribe or Territory****] of [****Insert city, county, state, tribe or territory name****].
