This Software License and Application Service Provider Agreement (hereinafter referred to as “Agreement”) is entered into by and between INSERT VENDOR NAME, a INSERT STATE OF INCORPORATION corporation having its principal place of business at INSERT STREET ADDRESS FOR VENDOR (hereinafter referred to as “Licensor”), and Mississippi Department of Information Technology Services having its principal place of business at 301 North Lamar Street, Suite 508, Jackson, Mississippi 39201 (hereinafter referred to as “ITS”), as contracting agent for the Mississippi Department of Human Services, located at 750 North State Street, Jackson, Mississippi 39202 (hereinafter referred to as “Licensee” and/or “MDHS”). ITS and MDHS are sometimes collectively referred to herein as “State.”
