COORDINARE has been established to take up the challenge of delivering the Primary Health Network for South Eastern NSW. COORDINARE provides a unique blend of private and public perspectives and innovative thinking which aims to transform the health of people in the region. HealthPathways is an online tool for health professionals providing assessment, management and local referral information. HealthPathways originated in New Zealand, with many Australian PHNs now implementing their own localised version of the program. It has been shown that implementing care pathways reduces the variability in clinical practice, improves quality of care, optimises the use of resources and improves patient outcomes. HealthPathways is both a process and an outcome: The process involves gathering together groups of clinicians relevant to a specific health service or condition.
