Upon acceptance of this application by Talley Fitness Center, I hereby agree to and accept the terms and conditions of the membership agreement printed on the reverse side of this application. The undersigned shall receive the membership rights and privileges and agrees to abide by all rules and policies of the Talley Fitness Center which are subject to change and which, in the opinion of the Management are deemed necessary and reasonable for the best interest of its members and the Talley Fitness Center.
