This Standard Form of Agreement (the “Agreement”) for energy conservation improvements to be completed at various facilities within the Warren County Career Center under the House Bill 264 Energy Conservation Program, identified as School Performance Contract (HB-264) Program Approval SN# 1412 (the “Project”), is made effective this th day of October, 2019, by and between the Vista Consulting Group, LLC (“VISTA”), P.O. Box 927, Lebanon, Ohio 45036 and The Board of Education of the Warren County Career Center (the “Owner”), 3525 SR 48 N., Lebanon, OH 45036, for the services and work described in the attached Scope of Work labeled Attachment A, pursuant to the terms and conditions contained herein.
