Reference is made to the UltraViolet License Agreement by and between the Executing Licensee Entity identified below (“Executing Licensee” or “you” or “your”) and the Digital Entertainment Content Ecosystem (DECE) LLC (the “Agreement”). The completion and execution of this UltraViolet Enrollment Form (the “Enrollment Form”) is required at the time of execution of the Agreement and in the event of any updates to the Licensed Roles or Licensed Territories. Upon execution hereof by both Parties, the “Licensed Roles” and “Licensed Territories” indicated below shall constitute the Licensed Roles and Licensed Territories under the Agreement. Please note that this form will need to be updated in the future to reflect any new Licensed Roles or Licensed Territories added. This Enrollment Form is hereby incorporated into the Agreement by reference. All capitalized terms not defined herein shall have the meaning set forth in the Agreement.
