I, the undersigned participant, desire to participate voluntarily in EZONE gaming activities at the University of Wisconsin – La Crosse EZONE-Esports & Gaming Facility. In consideration of the services of the State of Wisconsin, the Board of Regents of the University of Wisconsin System, and the University of Wisconsin-La Crosse, their officers, agents and employees, and the UW-La Crosse EZONE-Esports & Gaming Facility, (hereinafter collectively referred to as "UWL.EZONE"), I hereby agree to release and discharge the UWL.EZONE, on behalf of myself, my children, my parents, my heirs, assigns, personal representative and estate as follows:
