This Agreement is entered into between the Wisconsin Special Supplemental Nutrition Program for Women, Infants and Children (WIC Program) of the State of Wisconsin Department of Health Services, Division of Public Health (Department), and the Vendor. This Agreement will be for a period beginning November 1, 2019, or the initial authorization date, and will expire on October 31, 2022. The Department will terminate the Agreement if it determines that the Vendor has provided false information in connection with its application for authorization. Either party may choose to terminate the Agreement for cause after providing not less than 15 days advance written notice, subject to any applicable appeal rights under Wis. Admin. Code ch. DHS 149.
