The Health and Human Services Commission (“HHSC” or “System Agency”) and Helen Farabee Centers (“Grantee”), collectively referred to as “Parties” to HHSC Contract No. HHS000128700001 effective June 1, 2018 (the “Contract”), now want to amend the Contract further. Under the terms and conditions of the Contract, HHSC has been funding Grantee’s delivery of services under the Recovery Support Services-Treatment Organizations Texas Targeted Opioid Response (“RSS-TO TTOR) Program (the “Services”).
