The Health and Human Services Commission (“HHSC” or “System Agency”) is the Receiving Agency and The University of Texas Health Science Center at Tyler (“Contractor”) is the Performing Agency, parties to the Interagency Cooperation Contract, effective on September 1, 2015, and denominated Contract No. 2016- 048310-001C (“Contract”), as amended, now desire to further amend the Contract. This Amendment will be denominated as Contract No. 2016-048310-001D.
