(CCPR), Helpline understand that any information (written, verbal or otherwise) obtained during the performance of my duties must remain confidential including, but not limited to, the following: all information pertaining to helpline callers, program participants and their families; any information obtained from the caller or participant during the phone interview; any referrals or contacts with treatment facilities or medical providers.
