THIS AGREEMENT FOR RESIDENCY APPOINTMENT ("Agreement"), made this day of 2021 (“Effective Date”), by and between RESIDENT NAME, (“Resident”) and Trinity Health-Michigan, a Michigan nonprofit corporation, d/b/a ST. JOSEPH MERCY ANN ARBOR, located at 5301 East Huron River Drive, Ann Arbor, Michigan 48106, (“Hospital”). Resident and Hospital are individually referred to herein as a “Party” and sometimes collectively referred to hereinafter as the “Parties.”
