THIS BUSINESS ASSOCIATE AGREEMENT (“AGREEMENT”) is entered into by and between Florida Healthy Kids Corporation, a Florida non-profit corporation, (“FHKC” or “Covered Entity”) and [Vendor] (the “BA”) (collectively referred to as the “Parties”), and is incorporated in the contract for Dental Services and Coverage between FHKC and BA (hereby referred to as the “Contract” for purposes of this Agreement).
