(Initials) For Parent/Guardian of Minor or Counselor. I give permission for Participant to participate in (hereafter “the Program”). I acknowledge that the risk of injury from the activity involved in the Program is significant, including the potential for permanent disability and death, and while particular protective equipment and personal discipline will minimize this risk, the risk of serious injury does exist. I understand and appreciate the nature of such hazards and risks.
