This is an agreement between Frontier Utilities Northeast, LLC (“Frontier”, “us”), and the customer (“Customer”, “you”). These terms and conditions together with your Uniform Disclosure Statement (“UDS”) and/or Natural Gas Product Summary, and Rate Plan Summary (if applicable), which are incorporated herein by reference, constitute the agreement between you and Frontier (the “Agreement”). Frontier is licensed by the Illinois Commerce Commission (the “Commission”) as an alternative gas supplier and an alternative retail electric supplier to sell natural gas and electric service in Illinois. Subject to the terms and conditions of this Agreement, Frontier agrees to sell and deliver, and you agree to purchase and accept the quantity of gas and/or electricity, as measured or estimated by your Natural Gas Utility (“NGU”) and/or Electric Utility (“EU”), necessary to meet your residential or small commercial requirements based upon consumption data obtained by Frontier and upon the natural gas
