THIS MASTER SERVICES AGREEMENT (“Agreement”) made and entered into this _ _ day of , 20 (“Effective Date”), between UTILITY SERVICE CO., INC., whose primary place of business is 535 Courtney Hodges Boulevard, P.O. Box 1350, Perry, Georgia, 31069 (“SUPPLIER”) and the CITY OF PALM COAST, a municipal corporation of the State of Florida, holding tax exempt status, whose address is 160 Lake Avenue, Palm Coast, Florida 32164, (“CITY”). CITY and SUPPLIER are collectively referred to herein as “Parties”.
