A requirement for enrollment in childcare services for my child/children is that I understand and agree to the following attendance standards. I will ensure that my child/children attend(s) childcare on a regular basis and meets the attendance standards for childcare services. The standard consists of the child/children maintaining an absence rate of less than 40 days. After 40 days of absences, the child/children may be terminated from the childcare services program. The provider will notify me when my child/children is absent for five (5), 10, 15, 20, 25, 30, 35 and 40 consecutive days.
