Please read it carefully before the evaluation. When you sign this document, it will represent an agreement between you and the May Evaluation Center (MEC). Read it in advance and bring any questions you may have at the time of the evaluation. The evaluation cannot proceed until you have fully read, understood, and signed this agreement.
