THE DEPARTMENT OF STATE HEALTH SERVICES (“System Agency”), an administrative agency within the executive branch of the State of Texas, and VERITRACE, INC. (“Contractor”), having its principal office at 9487 Dry Fork Rd., Harrison, Ohio 45030 (each a “Party” and collectively the “Parties”), enter into the following agreement (“Contract”) for the production of vital statistics documents as required by the Texas Legislature for distribution by the State Registrar (the “Contract”).
