HEALTH SCIENCE CENTER (“Contractor”), who are collectively referred to herein as the “Parties,” to that certain contract for the COVID-19 Vaccine Outreach and Education Grant Program, effective June 3, 2021 and denominated as Contract No. HHS001043100001 (the “Contract”), now desire to amend the Contract.

The Contract is effective on November 1, 2020 and expires on August 31, 2022, unless renewed, extended, or terminated pursuant to the terms and conditions of the Contract. The Contract may be renewed for up to one additional one-year period, with renewal initiated at the sole discretion of System Agency. The System Agency, at its own discretion, may extend this Contract subject to terms and conditions mutually agreeable to both Parties. Continued funding of the Contract in future years is contingent upon the availability of funds and the satisfactory performance of the Grantee during the prior Contract period.
