This Ph.D. Survey Agreement (“Agreement”), made and effective as of DATE , between NAADAC, the Association for Addiction Professionals, and NAME OF STUDENT (“Ph.D. student”) of NAME OF COLLEGE/UNIVERSITY , an accredited educational institution, outlines the below terms and conditions for the distribution of a Ph.D. dissertation-related survey to NAADAC’s membership and/or constituents.
