THIS AGREEMENT to Obtain and Maintain Physical Damage Insurance on Buyer’s Motor Vehicle (“Addendum”) between CNAC - DPAC Acceptance (“Creditor”) and SARAHSERVICE FORMS (“Debtor”) is entered into effective as of 15th day of December, 2021. The Addendum is attached to and becomes part of your Retail Installment Contract and Security Agreement (“Contract”). The Addendum modifies the terms in your Contract. To the extent terms of the Addendum conflict with the Contract, the language in the Addendum controls. Please read it carefully. By signing this Addendum to the Contract, the Debtor and Creditor agree to be bound by the terms and conditions set forth herein.
