This BEHAVIORAL HEALTH SERVICES AGREEMENT (“Agreement”) is entered into and effective as of the effective date on the signature page (“Effective Date”). It is between Virginia Premier Health Plan, Inc., a Virginia corporation (“PREMIER”), and Behavioral Health Services Provider identified on the signature page of this Agreement (“BEHAVIORAL HEALTH PROVIDER”).
