This amendment incorporates the mutually agreeable Escheat Services Agreement, dated July 14, 2016, between the State of Ohio and Aetna. In addition, this amendment advises of a change to the OAKS Vendor Number for Aetna. Lastly, this amendment adds page 4 for the Summary of Amendments.
