THIS CLINICAL TRIAL AGREEMENT together with all Exhibits and Attachments, constitutes the (“Agreement”) that is made as of the day of 2013 (the “Effective Date”) by and between (Sponsor Name and Address) “Sponsor”; and the University of Connecticut Health Center, located at 263 Farmington Avenue, Farmington, CT 06030, identified above and on the signature page as (“Institution”). Sponsor and Institution may be collectively referred to as parties or singularly referred to as party.
