If any revisions are requested, they need prior authorization by the Assistant Superintendent of Business (CBO).2. Ensure there is an accurate and complete description of theContractor’s Scope of Services. Relevant portions of Contractor’s proposal (i.e. detail regarding services) can be excerpted and inserted, but Contractor’s entire should not be attached. In no circumstancesshould any “terms and conditions” from Contractor be inserted into the agreement.3. Review the insurance requirements for the person or entity and revise the insurance provisions of the agreement accordingly.4.
