This Agreement ("Agreement") dated 20 , (“Effective date”) between ("Agent" or “Agency”) and Worldwide Insurance Services, LLC (Worldwide Services Insurance Agency, LLC in California and New York) ("Company"), sets forth the terms and conditions under which “Agent” and “Agency” shall perform certain services for the Company. The designations “Agent” and “Agency” shall be interchangeable based upon the contracting party designated by signature below. Agent and Company may also be referred to individually as a “party” and together as the “parties.”
