Anthem Employer Group Inquiry Usage Agreement

This Usage Agreement is directed at the The User. The term “User” refers to the Group Health Plan listed below that has elected to use the Anthem Employer Group Inquiry application to access eligibility information and to perform a number of other functions on behalf of covered employees and their dependents. The Employer Group Inquiry application shall only be used by individuals who are employees of the Group Health Plan (as defined in section 3(1) of the Employee Retirement Income and Security Act of 1974 (ERISA), 29 U.S.C. 1002(1)) and is not intended for use by individuals who are employees of the Employer Group or plan sponsor. The Employer Group Inquiry application shall not constitute a HIPAA 834 or other HIPAA transaction.
