This Health Insurance Portability & Accountability Act Business Associate Addendum (“HIPAA Addendum”) is an addendum to the Service Agreement (and incorporated therein by reference) by and between Benefit Express Services, LLC (“BE” or “Business Associate”) and Client (“Client” or “Covered Entity”).
