This Maryland Participating Pharmacy Agreement (“Agreement”) is made by and between MC-21 Healthcare, LLC (herein “MC-Rx”), organized under the laws of Puerto Rico, with its principal place of business located in Angora Industrial Park, Lot 4, Road #1, Km 33.3 Barrio Bairoa, Caguas, PR 00725 and the undersigned Pharmacy (herein “Pharmacy”), collectively referred to as the “Parties”, to be effective _ (“Effective Date”), for the purpose of setting forth terms and conditions under which Pharmacy Services shall be rendered.
