THIS BROKER AGREEMENT (the “Agreement”) is entered into by and between Children’s Community Health Plan, Inc. (“CCHP”), and the agency set forth on the signature page of this Agreement (“Broker”), effective as of the date of signature by both parties hereto. Broker desires to sell benefit programs offered by CCHP, and CCHP desires to contract with the Broker to sell such programs.
