This Contract Extension is made and entered into this 30th day of June, 2021 by and between the Workforce Development Board of Flagler and Volusia Counties, Inc., d.b.a. CareerSource Flagler Volusia (hereinafter CareerSourceFV) a Florida private not for profit corporation, whose principal address is 329 Bill France Blvd. Daytona Beach, Florida 32114, and Flagler County Public Schools and George Washington Carver Foundation, Inc. (hereafter referred to as Contractors), whose principal addresses are 1769 East Moody Blvd Bldg. 2, Bunnell, FL 32110 and extends the original Contract between parties, including any and all Amendments and modifications to the original Contract dated the 30th of June 2020.
