This California Health Benefit Exchange Small Business Health Options (SHOP) Program Supplement Rider (the “Supplement”) supplements that certain Group Subscriber Agreement (the “Agreement”) between Health Plan or Insurance Issuer (HEALTH PLAN) and GROUP.
