THIS SUPPORT AGREEMENT, including all exhibits and schedules, is made and entered into by and between MANAGED ACCESS TO CHILD HEALTH, INC. d/b/a/ PARTNERSHIP FOR CHILD HEALTH (“THE PARTNERSHIP” herein) and National Alliance on Mental Illness – Jacksonville, Florida (“AGENCY” herein), and is made effective as of February 1, 2020 for the purpose of defining the roles and responsibilities of THE PARTIES in areas of common interest related to the Substance Abuse and Mental Health Services Administration (SAMHSA), Expansion Grant – System of Care Initiative Program including ongoing collaboration on family and youth engagement and the development of a new responsive web site for NAMI Jacksonville.
