This Agreement made this day of 2021 (“Effective Date”) is made by and between the International Sanitary Supply Association, an Illinois non-profit corporation with its principal place of business at 3300 Dundee Road, Northbrook, IL 60062 (hereinafter “ISSA”), and COMPANY, DESCRIPTION, STREET ADDRESS, CITY, STATE (hereinafter “Company”) (collectively referred to as the “Parties” and each individually as a “Party”).
