This will serve as a Letter of Agreement between “joint provider” and the University of Washington School of Medicine, to set the terms and conditions in the management of this UW Department of Medicine course. To assure that this meets all of the requirements of Category 1 credit, we are pleased to offer accreditation under the following terms and conditions. The University of Washington School of Medicine reserves the right to remove accreditation should the terms and conditions of this letter of agreement not be met.
