THIS AGREEMENT, by and between Georgia K9 Training (PROVIDER) and (Client) is entered into this (Date) WHEREAS, (Client) wishes to obtain a dog (“Service Dog”) to assist him/her with a disability that has been recognized by (Client)’s doctor; and PROVIDER wishes to assist (Client) in this endeavor; NOW, THEREFORE, the parties agree as follows:
