The Individual Investigator Agreement (IIA) is to be submitted to the IRB through IRBNet, with Appendix R: Adding an Unaffiliated Independent Individual, as part of the initial submission or an amendment, at which time the DU IRB will assess whether it will serve as the IRB of Record for an Unaffiliated Investigator (UI). The UI may not engage in the conduct of Human Subject Research until a signed copy of this agreement is returned to the DU Principal Investigator.

The Individual Investigator Agreement (IIA) is to be executed after the DU IRB has reviewed the Appendix Q: Individual/Unaffiliated Investigator Agreement Approval Form and has agreed to serve as the IRB of Record for an Unaffiliated Investigator (UI). The UI may not engage in the conduct of Human Subject Research until a signed copy of this agreement is returned to the DU Principal Investigator.
