This HIPAA BAA is effective as of September 4, 2019 (“Effective Date”), which is the date Customer indicated its acceptance of this HIPAA BAA electronically. This HIPAA BAA was electronically signed by Matthew Grant, Clinical Psychologist/Owner on behalf of Customer on the Effective Date.
