Oak Street, Juneau, WI 53039, and Greater Watertown Community Health Foundation, a 501(c)3 nonprofit, charitable foundation organized as a nonstock corporation pursuant to Chapter 181 of the Wisconsin Statutes with a principal place of business located at 600 East Main Street, Suite 200, Watertown, WI 53094 (“Foundation” or “Grantee”), collectively, the “Parties”.
