THIS SCHOOL BASED HEALTH CENTER AGREEMENT (this “Agreement”) is made as of the ___ day of __________ (the “Effective Date”), between the KILLINGLY board of education (the “Board”), and GENERATIONS FAMILY HEALTH CENTER, INC. (the “Clinic”) with respect to the use of space for and the operation of the school-based health center for children identified herein (the “Center”).
