DBA Triarq Practice Services with its principal office at 1050 Wilshire, Suite 300, Troy, Michigan 48084 ("BUSINESS ASSOCIATE"). The purpose of this Agreement is to comply with the requirements of the Health Insurance Portability and Accountability Act of 1996 and its implementing regulations (45 C.F.R. Parts 160-64), any applicable state privacy laws, any applicable state security laws, any applicable implementing regulations issued by the Insurance Commissioner or other regulatory authority having jurisdiction and the requirements of the Health Information Technology
