, a Managed Long Term Care Plan (“MLTC Plan”) and [Insert Name of Health Home] (“Health Home”). The purpose of this Agreement is to set forth the terms under which MLTC Plan will subcontract with Health Home to provide certain MLTC Plan care management services on MLTC Plan’s behalf to class members in the consolidated matters of U.S. v. State of New York, 13-CV-4165 (NGG) and O’Toole v. Cuomo, 13 CV-4166 (NGG) (E.D.N.Y.) (“U.S. v. State of New York”), who are enrolled in both MLTC Plan and Health Home (“Class Members”). This agreement applies only to Class Members and is not intended to alter the role or responsibilities of MLTC Plan or Health Home with regard to services to non-Class Members.
