The St. Andrews Club, Inc. (the “Club”), a corporation organized under the Florida Not-for-Profit Corporation Law, owns and operates a Fitness Center (the “Fitness Center”) on the Club’s property at 4475 North Ocean Blvd., Delray Beach, Florida for the enjoyment of the Club’s members, their immediate family (including children, grandchildren, and parents) and other guests. In consideration of, and as a condition to, the use of the Fitness Center by the undersigned (the “Guest”) who is a guest of the Member of the Club whose name is set forth below, the Guest has executed and delivered to the Club this Guest’s Fitness Center Assumption of Risk, General Release, Indemnification, and Covenant not to Sue Agreement (this “Agreement”) in which the Guest acknowledges and expressly agrees on behalf of himself or herself, and his or her heirs, executors, administrators, successors. and assigns (all such persons being, collectively, the “Releasing Parties”) that:
