(“Provider”), having a principal place of business at 1545 Sawtelle Blvd., Suite 36, Los Angeles, CA 90025 and the City of Coalinga, California, a municipal corporation (“Subscriber”) and is effective upon signature by the Subscriber (the “Effective Date”).
