Common use of Your Physician Clause in Contracts

Your Physician. ▇▇. ▇▇▇▇▇ shall be your primary treating physician. You understand, however, that ▇▇. ▇▇▇▇▇ may not be available from time to time due to illness, continuing medical education obligations or customary vacation periods. During any such unavailability, ▇▇. ▇▇▇▇▇ will designate a covering physician or other licensed medical professional to attend to your medical needs. The covering practitioner will have access to your medical history and course of care, but may only provide Professional Medical Services (without Amenities and Enhancements) to you during the period ▇▇. ▇▇▇▇▇ is unavailable. You also agree that in the event ▇▇. ▇▇▇▇▇ becomes unavailable during the term of this Agreement due to illness or other disability, you will not be entitled to a refund of any portion of the Membership Fees previously paid by you.
