Year-end Financial Report. Within 90 days of the close of CONTRACTOR’s fiscal year, the CONTRACTOR shall submit to COUNTY: 1. An annual statement of revenue and expenditure totals by account in accordance with the chart of accounts and reimbursement terms of this Agreement. COUNTY recognizes that the annual statement of revenue and expenditures will be an internally prepared system statement and will not be audited. However, the COUNTY reserves the right to request CONTRACTOR to provide audited financial statements. Expenses to provide audited financial statements shall be shared equally, i.e., 50% / 50%, between CONTRACTOR and COUNTY. If the cost to perform an audited financial statement would create an adverse financial impact upon CONTRACTOR, CONTRACTOR and the COUNTY agree to negotiate in good faith a revision to the fee structure accordingly. 2. Additional information to include: a. CONTRACTOR’s general ledger for local operations. b. Accounts receivable activity, patient ▇▇▇▇▇▇▇▇ and detailed support for all adjustments and write-offs. c. Detailed information and support documentation for all financial reports. d. Detailed activity and accounting information and supporting documentation for any revenue generated by personnel and equipment expensed in this Agreement.
Year-end Financial Report. On or before the end of each contract year and consistent with the timeline shown in Exhibit F, SEMSA shall submit a Year-end Reviewed Financial Statement to the Ambulance Enforcement Officer. This report shall include annual financial statements reviewed by an independent public accounting firm in accordance with generally accepted accounting procedures. Statements shall be available to the Ambulance Enforcement Officer within one hundred eighty (180) calendar days of the close of each fiscal year. If ▇▇▇▇▇’s financial statements are prepared on a consolidated basis, then separate balance sheets and income statements for the Lassen County operation shall be required and shall be subject to the independent auditor’s review. SEMSA shall make all financial records for Lassen County contract. ▇▇▇▇▇ agrees that all revenue generated using personnel or equipment expensed as described in this Agreement, as well as any subsidy paid by any government entity within Lassen County will be credited to Lassen County revenues
Year-end Financial Report. Within 120 days following the end of Manager’s fiscal year, the Manager shall provide an actual year-end revenue and expenditure report to the City Manager.
