CONTRACTOR who performs work at or for INSTITUTION are required to file evidence of Worker’s Compensation coverage or submit the "Notice of Designation as Independent Contractor" form certification of independent contractor with the Rhode Island Department of Labor and Training Workers’ Compensation under the RI Workers Compensation Law.
